Family Centers Health Care Earns National Quality Accolades
Greenwich, Conn. (September 11, 2024) – Family Centers Health Care (FCHC), a federally qualified health center serving public housing residents and families living in the greater Greenwich area, was recognized for its outstanding quality by the Health Resources and Services Administration (HRSA).
HRSA recently announced the recipients of its 2023 Quality Improvement Awards, celebrating the top-performing health centers nationwide and those that have made significant quality improvements over the past year. HRSA uses Uniform Data System (UDS) data to assess the impact and performance of health centers and promote data-driven quality improvements. FCHC was awarded five Quality Recognition badges, the most given to any health center in Connecticut. In addition, FCHC maintained its Person-Centered Medical Home status from the Joint Commission.
“We are extremely grateful for this recognition from HRSA for the high level of care we provide,” said Dennis Torres, Family Centers’ Chief Health Officer. “In a year when we more than doubled the number of patients served under our FQHC, these results demonstrate our commitment to delivering top-tier medical care. Despite the increased patient volume, our quality outcomes remained excellent, which is truly impressive.”

Health Center Quality Leader – Recognizes health centers that improved quality of care between 2022 and 2023.

Access Enhancer – Recognizes health centers that increased the total number of patients served and the number of patients receiving comprehensive services between 2022 and 2023.

Health Disparities Reducer –Recognizes health centers that demonstrate improvement in low birth weight, hypertension control, and/or uncontrolled diabetes outcomes between 2022 and 2023.

Advancing Health Information Technology (HIT) for Quality – Recognizes health centers that optimized HIT services for advancing telehealth, patient engagement, interoperability, and collection of social determinants of health to increase access to care and advance quality of care between 2022 and 2023.

Addressing Social Risk Factors - Recognizes health centers that collect data on patient social risk factors and increase the number of patients who receive enabling services between 2022 and 2023.

Patient Centered Medical Home – Recognizes health centers with patient centered medical home (PCMH) recognition in one or more delivery sites.
