Primary Care

Family Centers Health Care is licensed by the State of Connecticut and receives funding from the U.S. Department of Health and Human Services (HHS). It holds Federal Public Health Service (PHS) deemed status, which extends to certain health or health-related claims, including medical malpractice claims, for the organization and its covered individuals. Primary Care

In partnership with Yale New Haven Health and Greenwich Hospital, our primary care providers provide comprehensive care for patients of all ages, including specialized care for women and seniors. At our Greenwich Health Center, we offer preventive care, including checkups and immunizations, next-day visits for common illnesses, and support for chronic conditions like diabetes and high blood pressure.

Our Approach

We take a holistic, relationship-centered approach to care. Your primary care provider is your main point of contact for any health concern. They are supported by a team of nurses, social workers, specialists, and other medical professionals who help manage all aspects of your care, follow up on needs, and support your long-term health.
